Comments:
An unannounced monitoring inspection was conducted today between the hours of 9:40am and 1:15pm. At the time of entrance, 43 children were in care with 2 staff members present. The physical space, emergency supplies, attendance records, injury reports, medications, the Emergency Preparedness Plan, 5 children's records and Staff certification for CPR/First Aid and Mat training were reviewed. The children were observed playing games, working on projects and playing on the computer and tablets. Morning snack and handwashing was also observed. The children were served Cheerios and milk. Areas of non-compliance are identified in the violation notice. A Subsidy Health and Safety Inspection (SHSI) was conducted on the same date as this inspection. Based on observation and staff interview, the center was found to be out of ratio.
Evidence: Upon arrival at 9:40am, there were 43 children present with 2 staff. Staff reported the center was out of ratio and that another staff was on their way to the center. The staff member arrived at approximately 9:45am.

Based on record review, 2 of 5 injury reports did not contain all required information.
Evidence: 1. The injury report for Child #1 did not contain the time of injury. 2. The injury report for Child #2 did not contain the time parents were notified of the injury.

We usually have a second person to review our reports to make sure all information is complete.
