Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on 6/12/2020 and concluded on 6/12/2020. The designated person in charge was contacted by telephone to initiate the inspection. There were 0 children present and 3 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-665-520-B-11
Description: Based on record review, the center did not ensure that a written allergy care plan, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction, was obtained for each child with a diagnosed food allergy.

Evidence:
1. Child #1 has a diagnosed food allergy. The record for Child #1 did not contain an allergy care plan.
2. Child #2 has a diagnosed food allergy. The allergy care plan in the record for Child #2 did not contain a physician's signature.

Plan of Correction: We will make sure a plan is made and all plans are signed by parents, physician, and a staff member.

Description: Based on record review, a record of Lockdown practice drills were not maintained for one year.

Evidence:
Documentation of lockdown procedures practiced in 2019 were not available for review.

Plan of Correction: We will make a copy of drills before sending them to our main office.
