June 26, 2024 A review of staff personnel files was conducted this date by inspectors Diana De Silva, Miranda Wright, Leah Pagala, Derek Acosta and Margaret Allworth at the Pennino SACC office building. Record reviews for regional supervisors and subs are documented with Shrevewood site file.
The center did not undergo a Central Registry background check for employees every five years.
Evidence: Staff #1's Central registry results expired as of 12/11/2023.
Plan of Correction:
Submitted request for renewal of Central Registry Check.
