Comments:
An unannounced monitoring inspection was conducted today. There were 49 children in care, with a total of 4 staff members assigned to their direct supervision. Children's records, children's medications, the physical plant, classroom programming, administrative records, and emergency preparedness items were inspected today.
The children were observed upon arrival to the after school program. Staff were observed greeting each child and inviting them to participate in a variety of classroom activities.

Based on a review of children's health records, it was determined that four records do not include an immunization record. Evidence: Child records A, B, C, and F do not include documentation of a shot record. Each of these children have already entered care.

I will ask the school nurse for a copy of each child's health records.

Description:
Based on a review of children's medications, it was determined that one long term authorization was not fully completed. Evidence: The authorization for one child's long term non-prescription medication does not include a clear documented start/stop date for which the authorization is effective.

I will ask the parent to obtain the missing information.
