Comments:
An unannounced Monitoring Inspection took place on September 2, 2022 between the hours of 1:00 pm and 2:45 pm. There were 2 classrooms observed with a total of 16 children with 2 staff within the supervision guidelines. The children were observed drawing, painting, eating snack, and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log, medication, special care, class schedule, first aid kit and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Based on record review and interview, the written record of attendance was not maintained. Evidence: 1. At the time of inspection (9/2/22) 16 children were signed in and signed out on the attendance sheet. 2. The center supervisor acknowledge the written attendance was not maintained correctly.
Plan of Correction:
I will correct now and ensure this is completed correctly going forward.
