Comments:
An unannounced monitoring inspection was conducted on 9/28/2023 between 8:00am-8:40am. There were 22 children directly supervised by 2 staff. The physical plant, 3 staff records, 3 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed making bracelets and playing board games. Areas of non-compliance are identified in the Violation Notice. Based on record review, the center did not obtain documentation that one child received the immunizations required by the State Board of Health before the child can attend.
Evidence: 1. The record for Child #2 (start date 8/22/22) did not contain documentation of immunizations.
