Comments:
An unannounced monitoring inspection was conducted from 3:40 pm to 4:35 pm. There were 12 children in direct care and supervision of 3 staff members. The children were observed eating afternoon snack, joining the stamp club and going outside. Passports were being used for accountability. 5 children's records and 1 staff record was reviewed. Based on records review, the center did not obtain immunization records for children before the first day of enrollment. Evidence: 1. Child # 1's first day of enrollment was 8/26/19. 2. Child # 2's first day of enrollment was 9/14/19. 3. Immunization records were not obtained for both children before their first day of enrollment.

The records have been requested from the school office and we will remind them again.

Child # 2's first day of enrollment was 9/14/19. 3. Physical exam documentation was not obtained for both children within 30 days of enrollment.

The school was going to provide those to us, we will remind them again.
