Comments:
An unannounced monitoring visit was conducted today between the hours of 4:20 pm through 6:00 pm. At the time of entrance, 21 children were in care with 2 staff members present. The physical space, emergency supplies, evacuation drills, attendance records, Emergency Preparedness Plan, 5 children's records and Staff certification for CPR/First Aid and training were reviewed. Ratios were in compliance and required documents were posted. The children were observed during handwashing, snack time, playing games, and knitting. Areas of non-compliance are identified in the violation notice. A subsidy health and safety inspection (SHSI) was also conducted on this day. Based on record review and staff interview, 2 of 5 children's records did not contain all required information.
Evidence: 1. The record for Child #1 did not contain names of persons authorized to pick up the child. The record for Child #1 did not contain parent confirmation that the required information in the child's record is up to date. Staff stated they have requested documentation from the parent that the child's information is up to date but have not received the documentation at the time of this inspection. 2. The record for Child #2 did not contain the father's place of employment.
