Comments:
An unannounced monitoring inspection was conducted today from 2:30pm (prior to children's arrival) to 4:50pm. Children began arriving at approximately 4pm. There were 57 children and 5 staff members present. The physical space, emergency supplies, evacuation drills, attendance records, injury reports, medications, and the Emergency Preparedness Plan were reviewed. 5 children's records and Staff certification for CPR/First Aid, Mat training, and emergency information were also reviewed. Ratios were in compliance and required documents were posted. The children were observed participating in the following: handwashing, eating snack, completing homework, playing a variety of board games, and playing outside. Areas of non-compliance are identified in the violation notice. A subsidy health and safety inspection (SHSI) was also conducted today. Based on record review, 3 of 5 children's records did not contain all required information. Evidence: 1. The records for Child #1 and #2 did not contain documentation of names of persons authorized to pick up the child. 2. The records for Child #2 and #3 did not contain documentation of parent confirmation that the required information in the child's record is up to date annually. The most recent update for Child #2 and Child #3 available for review was dated 8/28/2018.
