A monitoring inspection was initiated on 09/09/2022 and concluded on 10/27/2022 to investigate a self-reported incident received by the department involving the area of supervision. Interviews were conducted with staff, documents were reviewed, and staff/child records were examined.

Based on staff interviews and a review of administrative documents, it was determined that the center's activities were not maintained in compliance with the center's own policies and procedures that are required.
Evidence: 1- The center has a written policy which states that "If any child cannot be accounted for contact the RS on Duty." 2 - When two children eloped, and were unaccounted for, the staff member in charge who supervised the children while another staff searched for the unaccounted, did not contact the RS on duty.
Plan of Correction:
Center staff reviewed the policies relating to safety, accountability, supervision and health. Center will ensure that their own policies on safety and accountability are followed.
