63.2 Child Abuse & Neglect

Comments:
A monitoring inspection was conducted on 5/23/2023 at 8:30am in response to a self reported incident where an eight year old child was left unattended on the playground on 5/17/2023.

Interviews were conducted with staff and children and staff records were reviewed.

Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-340-A
Description: Based on staff interview, the staff did not always ensure the care and protection of children while supervising children.
Evidence:
1. On May 17, 2023, 8 children were playing outside on the playground. Staff #1 lined up 8 children (including Child #1) at Door 7 to go inside at approximately 8:20am.
2. Staff #1 was unaware that Door 7 closed prior to Child #1 gaining entry to the building and locking Child #1 outside.
2. Upon entering the building the children proceed directly to the bathroom to wash their hands, after which the teacher ensures the presence of all assigned children.
3. While the children assigned to Staff #1 were washing their hands, Staff #2 observed Child #1 run past the SACC classroom window and ring the doorbell at the SACC entrance to gain entry back into the SACC program.
3. Child #1 was admitted back into the building and reunited with program staff approximately 1-2 minutes after being separated from the group.

Plan of Correction: All staff will follow the SACC program policies and procedures for children?s arrival and departure to ensure the safety, supervision, and accountability of children at all times. Hutchison center staff will receive additional training on SACC?s Safety, Accountability and Supervision procedures, including active supervision and the consistent and focuses implementation of procedures for attendance and checklists during arrival, departure, activity time, transitions, and name to face roster checks. The center specific safety and accountability plan will be reviewed and updated. The Regional Supervisor (Josh McQuiston) will review the updated center safety and accountability plan and conduct periodic site visits to ensure the consistent implementation of program policies and procedures for attendance and accountability.
