Comments:
An unannounced monitoring inspection was conducted today beginning at 8:20 am. At the beginning of the inspection there were 12 children directly supervised by 2 staff. The physical plant, 3 staff required documents, 5 children?s records, 1 medication, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing video games and air hockey, and arriving to the morning program. Staff and children interactions were observed to be positive and constructive in nature. There was a variety of age appropriate materials for the children in care. Staff background checks, annual training, and qualifications are reviewed bi annually at the main office. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Based on record review, one of five children's records reviewed did not contain all required information. Evidence: Child 2's record did not contain the name, address, and phone number for 1 of the 2 required emergency contacts. One of the two provided emergency contacts is also listed as a custodial guardian.
