Comments:
An unannounced renewal inspection was conducted on 3/10/2023 from 7:53 am-8:50 am. There were 14 children supervised by 3 direct care staff. The physical plant, 4 staff records, 5 children?s records, medications, emergency supplies, and policies were inspected. Children were observed participating in group play and tabletop activities. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Based on record review and interview, the center did not obtain all the required information for each child's record.
Evidence: 1. Child #4's record did not have an annual update confirming the child's information in the child's record was up-to-date.
2. Child #4's record did not have a first date of attendance.
