Comments:
An unannounced monitoring inspection was conducted November 26 2019 between 8:00am and 9:30am. There were 35 children in the direct care of 3 staff members. Children were observed participating in a large group question game, going to breakfast, playing indoor basketball, and preparing for school. The center is clean and organized. There are age-appropriate materials for all age groups in care. Areas of non-compliance are listed on the violation notice. Based on record review, 1 of 5 children's records did not have all of the required documentation.
Evidence: Child #3 record does not have a complete record for the 2nd emergency contact.

Description:
Based on observation, 4 of 5 records of minor injuries did not contain all of required documentation.
Evidence: 4 of the 5 injury records reviewed only had 1 documented signature on the form. The standards require 2.

Will have the two people there at the same time to sign the forms.
