Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated and concluded on November 9, 2021. The supervisor was contacted by telephone and a virtual inspection was conducted. There were 24 children present, ranging in ages from 5 years old to 11 years old, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records did not contain all required information.
Evidence:
1. Child #1's record did not contain an address for one emergency contact.
2. Child #3's record did not contain documentation of an annual update.

Plan of Correction: The staff will get the missing information from the parents and update the record.

Standard #: 22VAC40-185-540-C
Description: Based on observation and interview, the required first aid kits did not contain required supplies.
Evidence:
1. The first aid kit was missing a First Aid Instructional Manual
2. Staff # 1 confirmed that the first aid kit was missing a First Aid Instructional Manual.

Plan of Correction: Staff will contact supervisor to place an order for a First Aid Instruction Manual.
