Comments:
An unannounced renewal inspection was conducted between 2:45pm-4:45pm today. There were 28 children present and four staff members supervising. Children were observed hand washing, eating snack, and transitioning to the playground. The physical plant, programs, children's records, medications, and emergency drills were also reviewed. Staff records are reviewed annually in the administrative office; however, Medication Administration Training (MAT) and First Aid and CPR certificates were available for review. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review and interview, the center did not inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention. Evidence: 1. According to an injury report dated 10/18/2021, Child #6 jumped off of a jungle gym located on the playground and hurt their right wrist. Child #6 was taken to the doctor's office where they were diagnosed with a "Bone Buckle" and received a cast for the right arm. 2. The department's representative was not notified of this injury within two days.
Plan of Correction:
Moving forward, staff will notify the department's representative within 48 hours of an injury that occurs while children are in the care of the school.

Based on observation, staff did not wash their hands before serving food or beverages. Evidence: 1. Staff #2 did not wash their hands before serving food or beverage. Staff #6 put on food service gloves, but did not wash their hands before serving children snack and beverage.

Before preparing or serving food or beverages, staff will wash their hands.

Description:
Based on medication review, the center has medication that does not have a prescription label in accordance with the prescriber's instructions pertaining to dosage, frequency, and manner of administration. Evidence: 1. Child #6 has emergency medication on site that does not have the pharmacy prescription label attached.
Plan of Correction:
The parents will be contacted to obtain a prescription label for the emergency medication.

Based on observation, the findings of the most recent inspection of the facility was not posted. Evidence: 1. The findings of the most recent inspection of the facility was not posted in a location to be easily read. The violation notice was located in a folder labelled "Licensing", but the violation notice was turned towards the wall and not revealing the contents.

The findings of the most recent inspection notice will be posted and displayed to show findings.
