Comments:
A Subsidy Health and Safety Inspection (SHSI) was conducted on 3/6/2020 to review supplemental health and safety requirements. 37 children were in care with 3 staff members present. The physical space, emergency supplies, evacuation drills, attendance records, injury reports, medications, the Emergency Preparedness Plan, 5 children's records and Staff certification for CPR/First Aid and Mat training were reviewed. Ratios were in compliance and required documents were posted. The children were observed playing games, reading, making bracelets, and coloring. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental
inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-550-2
Description: Based on review of injury reports, the vendor did not inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional.

Evidence:
1. Child #1 injured their wrist while under the vendor's supervision on 11/4/19.
2. Documentation in Child #1's record indicated that Child #1 was seen by their physician and was diagnosed with a sprained wrist.
3. The Department was not notified of the injury.

Plan of Correction: Moving forward, we will inform the Department of injuries requiring outside medical attention.
