Comments:
An unannounced VA Code Compliance monitoring inspection was conducted from 11:45 am - 12:45 pm. There were 20 children in care being supervised by 3 staff. The children were observed eating lunch.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of three staff records and interviews it was determined that there was not staff on site who had received daily health observation training. Evidence: The records for staff A, B, and C did not contain documentation of daily health observation training. It was reported by administration that staff had not received daily health observation training.
Plan of Correction:
Staff will receive daily health observation training.

Based on review of three staff records and interviews it was determined that not all records contained documentation of child abuse and neglect training. Evidence: The record for staff A (start date: 09/06/2022) and staff B (start date: 08/15/2015) did not have documentation of receiving child abuse and neglect training.

Description:
Based on review of three staff records it was determined that not all background checks were repeated every five years. Evidence: The most recent central registry check on file for Staff A is dated 10/23/2015. The most recent Sworn Statement or Affirmation form on file for Staff C is dated 10/18/2016.

A repeat central registry check will be obtained for Staff B. Staff C will complete a sworn statement or affirmation form.
