Comments:
An unannounced VA 22.1-289 Code Compliance monitoring inspection was conducted from 3:30 pm- 4:15 pm. There were nine children in care being supervised by one staff. The children were observed participating in independent activities. A total of three children's records and three staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of three staff records it was determined that not all records contained documentation of a staff health report. Evidence: The record for staff B did not have a staff health report on file.
