Comments:
An unannounced inspection was completed on 09/25/2019 from 9:50 A.M.-2:30 P.M. At the time of the inspection 52 children were in care with nine staff and two administrators present. Five children records, eight staff records, one medication and required documentation, five injury reports, evacuation drills, first aid kits, emergency supplies, playground, health inspection, fire inspection, buses used for transportation,and posted required information were reviewed. Children were observed during morning work cycle which included science activities, geography lessons, morning snack time, easel painting, handwashing, lunchtime, outdoor playtime, departure of half day students, and naptime. The toddler classroom was observed arriving back to the center from a field trip to the Apple Orchard. Exceptions to compliance are noted on the violation notice.

Based on observation, the center failed to ensure that equipment was maintained in a clean,safe, and operable condition.
Evidence: 1. On the playground there where was a green metal wheelbarrow, and a red metal wheelbarrow that was rusted and had paint peeling. 2. On the playground structure there were three board slates that had insect damage causing bolts to be exposed, and the wood was cracked and splintering. 3. Staff verified that the wheelbarrows were rusting and there were boards damaged and needing replacing.
Plan of Correction:
The wheelbarrows will be disposed of it they can't be repainted, and the boards will be replaced by maintenance.

Description:
Based on a review of background checks, the center failed to ensure that an out of state central registry background check by the end of the 30th day of employment.
Evidence: 1. Staff #3 began employment on 04/11/2019. This staff person indicated that she resided in one state other than Virginia within the past 5 years. The required out of state central registry search request for this staff person has not been submitted. 2. Staff verified that the record check had not been submitted within the required time frame.
Plan of Correction:
Administration will send the out of state central registry background check immediately.
