Comments:
An unannounced monitoring inspection was conducted today. There were 58 children in care, with a total of 10 staff members assigned to their direct supervision. Children's records, staff records, staff training, background checks, the indoor/outdoor physical plant, emergency preparedness items, children's nutrition, classroom programming, and classroom interactions were inspected today.
The children were observed playing color/shape matching games, engaging a wide variety of apple theme based activities, singing/dancing to music, engaging in circle time discussions, practicing shape recognition skills, playing outside, and engaging in classroom creative play activities.

Based on a review of children's records, it was determined that one record does not include all required information. Evidence: Child record B does not include the father's work phone number.

Based on a review of the emergency preparedness plan, it was determined that this plan does not include all required procedural components. Evidence: The emergency preparedness plan does not include "sounding of alarms" information.

Description:
Based on a review of staff records, it was determined that one required out of state search of the central registry was not requested. Evidence: Staff C began employment on 6/21/19. This person's file documentation indicates that she has lived outside of the state of VA within the past 5 years. Management staff did not submit a request for a search of this person's out of state central registry history.
Plan of Correction:
I will submit the search request and show documentation of the request to my Inspector within 10 days as required.
