Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 10/14/2020 and concluded on 10/16/2020. The director was contacted by telephone to initiate the inspection. There were 89 children present and 14 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on review of four children's records it was determined that not all records contained documentation of immunizations before the first date of attendance. Evidence: The record for child A (start date: 10/03/2019) did not have documentation of an immunization record on file.

Description:
Based on interviews, it was determined that the center did not ensure the care, protection, and guidance of children in care. Evidence: The center failed to adhere to the health and safety guidelines established due to the COVID-19 pandemic of ensuring that children were maintaining social distance. Evidence: Six children were observed in the two year old room sitting closely on the circle-time carpet playing with a variety of manipulative toys.

Based on interview and observation of the five year old room it was determined that the required staff to child ratio was not always maintained. Evidence: There were 11 children being supervised by 1 staff member in the five year old room. This five year old room is designated for Kindergarten-age children however, Child C (d.o.b 10/07/2015) does not meet the eligibility of turning five before the September 30 deadline.
