Discussion on staffing and supervision, physical plant, children and staff records
Comments:
An unannounced monitoring inspection was conducted today from approximately 9:00am-10:00 am. There were 46 children directly supervised by 9 staff. The physical plant, 4 staff records, 4 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. No new medications since the last inspection. Children were observed participating in group play, reading books, playing outside, and eating snack. There was an assortment of age appropriate materials for the children in care.

Based on staff records review, the center did not obtain documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children. Evidence: 1. Staff #1 (date of employment 5/11/2023) did not have tuberculosis screening at the time of employment and prior to coming into contact with children. Staff #1 was providing care for children during the inspection.

Based on observation, the center did not maintain areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: 1. In the two year old classroom, a blue couch had tears on the sides. 2. In the three and four year old classroom, a toilet had protruding screws in a bathroom accessible to children.
