Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on November 16, 2022, from 10:15 a.m. through 11:00a.m. Three staff records were reviewed and updated.
Areas observed were circle time, story, nd snack table disinfecting/preparation. This preschool is equipped with a wide variety of materials for the children in care.

Based on record review, the center failed to obtain a central registry check for one staff within 30 calendar days of employment.
Evidence:
1. Three staff records were reviewed and updated. 2. Staff #3 did not have documentation of obtaining a central registry check within 30 days of employment. Staff #3 date of hire was documented 11/15/2019. 3. Staff confirmed the central registry check had not been obtained for Staff #3.
Plan of Correction:
A central registry check will be completed for Staff #3. Documentation will be sent to the licensing inspector.
