Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and a virtual tour of the program.
A monitoring inspection was initiated on 7/29/2021 and concluded on 7/30/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were no children present. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 caregiver/household records were reviewed.

Based on interview, the provider did not obtain updated tuberculosis (TB) tests or screenings for each household member and caregiver every two years. Evidence: 1. The provider stated Caregiver #1, Household member #1, Household member #2 and Household member #3 had not received an updated Tuberculosis (TB) test or screening. Last date of test or screening was on 12/26/2018 for Caregiver #1, 7/14/2019 for Household member #1, 5/12/2019 for Household member #2, 5/12/2019 for Household member #3.
Plan of Correction:
Provider stated appointments have been made for all 4 with expired TB's for 8/18/2021.
