Comments:
A code compliance inspection was initiated on 3-1-2023 and concluded on 3-1-2023. There were 55 children present, ranging in ages from 2 to 4, with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, and staff to child ratios. A total of 10 staff records were reviewed.
Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the program.
Based on record review and staff interview the center did not ensure to obtain a copy of the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years. Evidence: Staff confirmed that Staff #7, first date of employment 4-26-2022, had not obtain an out of state child abuse and neglect record check for Arkansas where they had lived within the last five years.
Plan of Correction:
I have contacted ASP, received the proper background paperwork. Kate will complete it 3-9-23 and it will be mailed out on 3-9-23.
