An unannounced monitoring inspection was conducted beginning at 12:15 p.m. and ending at 2:00 p.m. with the head of school and office manager. There were eight children in care ages three and four years old under the care and supervision of 2 staff. Children were observed during lunch. Lunch is brought from home. Center documents and two staff records were reviewed.

Based on review of documentation and interview with staff, it was determined that the center did not provide required written disclosures to the parents and guardians of the children in the center. Evidence: There was no written disclosure regarding qualifications of personnel.
