Comments:
A mandated Monitoring inspection was conducted today. There were 24 children present with 4 staff and 2 parent volunteers. Children were observed in circle time, getting ready to go outside, washing hands for snack (blueberries, strawberries and muffins downstairs, and grapes, snap peas and crackers upstairs) and painting. Reviewed 3 staff files, 2 parent volunteer files and 4 children's files, emergency evacuation log and supplies, first aid kits, attendance, and physical plant. Standard #: 22VAC40-185-160-A
Description: Based on record review, an individual from an independent contractor did not submit documentation of a negative tuberculosis screening within 21 days of volunteering.
Evidence:
Volunteer #1 did not have documentation of a negative TB test.

Plan of Correction: I will have them get a copy

Standard #: 22VAC40-185-70-A
Description: Based on record review, documentation that two or more references as to character and reputation as well as competency were not checked before employment.
Evidence:
Staff#1 did not have documentation of 2 references being checked prior to employment.

Plan of Correction: I will have them done and put them in the file.

Standard #: 22VAC40-185-280-H
Description: Based on observation, potentially harmful agents were stored in purses accessible to children.
Evidence:
A Volunteer's purse was observed hanging by the children's coats, accessible to children.

Plan of Correction: This violation was corrected by the purse being made inaccessible to children.
