Comments:
A VA 22.1-289.031 code compliance monitoring inspection was conducted from 10:45 am - 11:45 am. There were 13 children in care being supervised by 4 staff. The children were observed participating in center activities and art projects. A total of four children's records and four staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of four staff records it was determined that not all staff had documentation of a staff health report on file. Evidence: There was no staff health report on file for staff A (start date: 03/14/24).

Description:
Based on observation it was determined that not all areas were free of hazards. Evidence: 1.) A large container of disinfectant solution was observed on the bathroom floor of the four year old room. 2.) A bottle of hand sanitizer was observed on a table in the three year old room accessible to children.
Plan of Correction:
The chemicals will be locked and kept inaccessible to children.
