Technical Assistance:
Discussion on staff records and hazardous materials. Site director was instructed to ensure that all staff be trained in prevention of shaken baby syndrome and abusive head trauma.
Comments:
A monitoring inspection was initiated on 5/4/2022 and concluded on 5/4/2022. There were a total of 87 children with 17 staff members present. Children were observed during snack time, coloring, engaged in group activities and indoor play time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 6 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on observation, equipment in the four year old classroom was not maintained in a safe condition.
Evidence: A wooden shelving unit was observed stacked on top of another wooden shelving unit, creating a risk for tipping over.

Evidence: Corrective fluid labeled keep out of reach of children, was found in an unlocked drawer in reach of children.
