The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.
Comments:
This monitoring inspection was initiated and concluded on February 22nd, 2021. The director was contacted by telephone to initiated the inspection. There were 23 children and 7 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children's records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice. Based on a review of children's records, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence: Child #1's record does not contain documentation of immunizations by the first day of attendance. Child's first day of attendance was 2/16/2020.
Plan of Correction:
I will contact parents to get documentation of immunizations on the first day of attendance.

Description:
Based on a review of staff records, the center failed to ensure that staff orientation training was not completed by the end of the first day.
Evidence: Staff #1 did not have documentation of orientation by the end of the first day. Staff #1's first day of employment was 10/08/2019 and orientation for this staff member was conducted 11/20/3019.
