Comments:
An unannounced monitoring inspection was conducted on September 15, 2023 from approximately 10:40 am to 1:40 pm. There were 74 children in attendance and a total of 15 staff present. 8 staff records and 6 children?s records were viewed. All areas of the center were observed including classrooms, playgrounds, and bathrooms. The children were observed playing outside, eating lunch, and taking a nap, while the licensing inspector was on-site.
Violations were cited as a result of this inspection.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice sign, and return it to me within 5 business days from today, 09/22/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).
Based on record review, the center failed to ensure that each staff shall undergo a criminal background check every five years. Evidence: The record for staff #6, date of employment 11/28/05, contained a criminal background check dated 06/18/18. A repeat criminal background check was due on or before 06/18/2023.
