An unannounced subsidy health and safety inspection was conducted on November 28, 2023 from approximately 11:50 am to 2:30 pm in response to a self-reported incident that was received and occurred on November 15, 2023. Documentation was gathered and interviews were conducted in person and via telephone.
Violations were cited as a result of this inspection.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from today, 11/30/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).
Based on record review, the center failed to ensure that staff who work directly with children shall annually attend the department's health and safety update course. Evidence: The record for staff #5, date of employment 12/12/22, contained an annual health and safety update course dated 07/14/2022. The updated course was due on 07/14/2023.
