Violations:
Standard #: 22VAC40-185-510-G
Description: Based on review of staff trainings, medications, medication logs, and interview with staff, the center did not ensure the medication administered by a staff member who meets the requirements.

Evidence: A review of the medication log revealed on 11/25/19, Medication #2 was administered by Staff#5 and the MAT certificate was dated 4/18/16. Interview with staff confirmed the MAT certification has expired, medication was administered, and MAT training needed to be rescheduled.

Plan of Correction: The program will ensure Staff is scheduled for MAT certification training as soon as possible. Another staff who is MAT certified will administer medication at this time.
