Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on Tuesday,June 16,2020 and concluded on Wednesday, June 17, 2020. The manager was contacted by telephone to initiate the inspection. There were no children and staff present as the program has been closed due to COVID-19. The inspector emailed the manager a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Based on review of children's record and interview with staff, the center did not ensure to obtain all required information for each enrolled child.
Evidence: In Child #1's record, the name, address, number for the second contact person to call in emergency if a parent cannot be reach was not available. Interview with staff confirmed there was only one designated contact available for review during this inspection.
Plan of Correction:
The Family Services Manager will review all files and follow up with family advocate to collect information prior to school year beginning.

Description:
Based on staff record and interview, the center did not obtain all information for staff records.
Evidence: In Staff#2's record (date of employment:10/24/17) there was documentation for one reference available for review. Interview with staff confirmed there was only one reference available during the inspection.

The Human Resources manager will complete a second reference check.

Description:
Based on review of staff records, documentation, and interview with staff, the center did not ensure to obtain a copy of results of an out of state search of the central registry.
Evidence: In Staff#1's record (date of employment: 12/18/19), a copy of results of a search of the out of state central registry was not available for review. Interview with staff confirmed the documentation for the results or follow up documentation was not available for review during this inspection.
Plan of Correction:
The program sent request for employee completion yesterday.
