Comments:
A renewal inspection was conducted January 16, 2024. The licensing inspector was onsite, completing the inspection from approximately 3:15pm until 5:00pm. There were 49 children present, ranging in ages from five to 12 years, with five staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five children's records and seven staff members? records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Based on review of five children?s records, the center did not ensure one child?s immunization records was not signed by a physician, his designee, or an official of a local health department. Evidence: The record for Child #1 (First day of attendance 08/21/2023) contained immunizations records that were not signed and dated by a physician.

Going forward, we will ensure all records from the doctor is signed and dated.

Description:
Based on observations, the center did not ensure all hazardous substances such as cleaning materials, were kept in a locked place using a safe locking method that prevented access by children. Evidence: Three cabinets in three classrooms contained unlocked cleaning chemicals. Children were observed in the three classrooms during the inspection.
Plan of Correction:
We will review the policies of keep cleaning chemicals and other hazards locked at all times when they are not in use.
