Comments:
A mandated monitoring inspection was conducted today. There were 10 children present with 2 staff. Children were observed crocheting, washing hands, playing age appropriate video games, using a tablet, and playing with legos. Reviewed 4 children's files, 2 staff files, emergency evacuation log and plan, first aid kit, attendance and physical plant. There were no medications on site. Based on record review, each child did not have a physical within 30 days of the first day of care. Evidence: Child#2 (start date 8/21/19) did not have documentation of a physical in their file for review.

Description:
Based on record review, the center did not maintain a separate record for each child enrolled that contained documentation of child updates and confirmation of annual up-to-date information. Evidence: Child#1 was missing documentation of annual update. File indicated last update was 2013. Staff stated the current file was brought to main office accidentally.

Description:
Based on observation, areas and equipment of the center are not being maintained in a safe, clean or operable condition. Evidence: Observed a cabinet with a hanging/broken door accessible to children.
Plan of Correction:
An initial work order was placed at the end of the last school year (approximately May 2019). The maintenance supervisor says it has been ordered and will be delivered soon.
