Comments:
An unannounced renewal inspection was conducted on 01/02/2020 from 11:20 am - 2:30 pm. At the time of the tour there were 16 children in care with four teaching staff and two administrative staff. The children ranged in age from infant - four years. Children were observed during lunch and nap. Records were reviewed for six children, five staff and five board members. Five medication reviews conducted. Violations were observed in two parts of the CDC standards and are listed on the violation notice.
Based on observation, the center failed to ensure that sandboxes with bottoms which prevent drainage shall be covered when not in use.
Evidence: The green plastic sandbox, located on the preschool playground, did not have a cover.
Plan of Correction:
Sandbox will be discarded if lid can not be located.

Evidence: 1. One container of over-the-counter medication was not labeled with the child's name. a. The medication was stored inside of a labeled plastic bag.
