Comments:
An unannounced monitoring inspection was conducted on 11/30/2023 from 10:35 am to 12:35 pm. There were 31 children in care, ages four months to four years, in four groups under the care of seven staff. The Licensing Inspector reviewed four children?s records, three staff records, and other program records. The children were observed in indoor play and getting ready for lunch.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review, the center failed to obtain documentation that a child had received the immunizations required by the State Board of Health before the child began attendance at the center.
EVIDENCE: Child 4 began attending the center on 10/10/2023 and documentation of immunization was not provided until 10/16/2023.
Plan of Correction:
From now on, no child will be permitted to begin attending the center until documentation of up-to-date immunizations is obtained.
