Comments: A complaint investigation regarding incidents which occurred in August 2019 regarding ratio, labeling of food and staff supervision was conducted on October 23, 2019 from 2:00pm to 3:00pm. Interviews and observations were conducted. Documentation was reviewed. There was not a preponderance of evidence therefore the complaint is not valid.
