Technical Assistance:
Standard 22 VAC 40-185-(8)-560-F was discussed regrading clear documentation of meals/ snacks served to children.
Comments:
An unannounced monitoring inspection was conducted on September 17, 2019. The inspection began at approximately 1:25 pm and ended at 3:30 pm. There were 19 children present at arrival to the facility with two staff members. An additional staff person arrived for support purposes. The children were observed during their rest period, eating snack and singing. No medications are currently at the center. Five children's records were reviewed. Areas of noncompliance are noted on the violation notice. Please contact the Licensing Inspector, Christine Mahan (757) 404-0568 with any questions.
Based on record review, in five of five children's records reviewed, the licensee did not ensure that each child's record included all required information.
Evidence: The following records listed below were determined incomplete. The missing information was verified by the center lead staff, Staff #1. 1) Child #2-There was no documentation of the employment phone number for one parent listed. 2) Child #1, #2, #3, #4 and #5- There was no documentation of previous childcare/ schools attended. 3) Child #1, #2, #3, #4 and #5- There was no documentation of any additional programs/ schools that the child is concurrently attending.
Plan of Correction:
Will get with Family Services and let them know this is missing from the new application and will have the forms updated.

Description:
Based on review, in three of three staff records reviewed, the licensee did not ensure all the required on site information was available for each staff.
Evidence: There was not documentation on site for Staff #1, Staff #2 and Staff #3 that included the name, address and telephone number of a person to be notified in an emergency and information about any health problems which may interfere with fulfilling the job responsibilities, Staff #1 verified this information was not at the center as required.

Will get with HR for updated information.

Description:
Based on observation and staff interviews, the licensee did not ensure the findings of the most recent inspection of the facility were posted.
Evidence: The documentation of the most recent inspection conducted in February 2019 was observed stored in a center's notebook. This was verified by Staff #1.

Will update it as soon as possible.
