Comments:
An unannounced monitoring inspection was conducted on February 27, 2020. The inspection began at approximately 10:10 am and ended at 12:15 pm. There were 38 children present at arrival to the facility with five staff members. The sample size of records reviewed included five children's records and the health related information and emergency contact information for the staff members on site during the inspection. The children were observed working on science experiments and coloring in journals. No medications are currently at the center. The resilient surfacing was not measured on the playground due to inclement weather. Areas of noncompliance are noted on the violation notice. Standard #: 22VAC40-185-40-I
Description: Based on documentation review and provider interviews, the licnesee did not ensure they had developed written procedures for injury prevention.

Evidence: Staff #1 confirmed there were no injury prevention procedures available for review.

Plan of Correction: The designated center supervisor get the plan and print and post it the center and place copy in the book.

Standard #: 22VAC40-185-70-A
Description: Based on review, in one of five staff records reviewed, the licensee did not ensure all the required on site information was available for each staff.

Evidence: There was not documentation on site for Staff #3 that included the name, address and telephone number of a person to be notified in an emergency and information about any health problems which may interfere with fulfilling the job responsibilities, Staff #1 verified this information was not at the center as required.

Plan of Correction: The designated center supervisor placed information in the record during inspection.

Standard #: 22VAC40-185-550-M
Description: Based on documentation review and staff interviews, the licensee did not ensure the center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and include all required information.

Evidence: Two of two injury reports available for review did not include how the parent was notified of the injury.

Plan of Correction: The designated center supervisor will start documenting it today.
