This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.
A complaint inspection was initiated on August 25, 2021 and concluded on September 7, 2021. A complaint incident was received by the department regarding allegations in the areas of supervision. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation. The preponderance of evidence does not support the allegations and the complaint has been deemed not valid.
