A complaint inspection was completed on 07/28/2022. A complaint was received by the department regarding allegations in the areas of: care, protection, and guidance; physical plant; and staff-to-child ration requirements. Observations and staff interviews were conducted during the inspection. The evidence gathered during the investigation supported the allegation of non-compliance with the standards or law. Based on observation and interview, the provider failed to ensure that staff-to-child ratios were maintained on 07/28/2022. Evidence: 1. Upon entering the infant room, the licensing inspector observed 5 infants (ranging from 2.5 months to 11.5 months) with 1 staff. The required ratio is 4:1. 2. Program Director acknowledged the classroom was out of ratio.
