Comments:
A monitoring inspection was conducted on Thursday, June 6, 2024 from 10:24 am to 2:01 pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. 83 children were present, ranging in ages from 5 months to 5 years with 18 staff supervising.
9 children?s records and 5 staff records were reviewed.
The inspector reviewed compliance in the areas of administration, staff qualifications and training, physical plant, staffing and supervision, programs, special care and emergencies, special services, and background checks.
During the inspection, children were observed napping and playing. Staff were observed having positive interactions with the children. The center is equipped with toys and supplies and items were available to the children.
Information gathered during the inspection determined non-compliance with an applicable standard or law and a violation was documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it within 5 business days. Please specify how the deficient practice will be or has been corrected.
Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on observation and interview, the center failed to ensure that two infant?s bottles were dated.
Evidence: The bottles in the refrigerator for C10 and C11 were not dated. In an exit interview on 6/06/24, S1 acknowledged that the bottles weren?t dated as required.
Plan of Correction:
The bottles were dated immediately. Teachers will check dates on all bottles upon the child's arrival. A message was sent to parents reminding them to label and date the bottles daily. Daily monitoring will be done by the director and the assistant director.
