8VAC20-780-560G All food from home, including water bottles if filled at home, will need to be labeled with name and date.
8VAC20-780-330B Play equipment exceeding 8 feet in height requires documentation from a reputable source with expertise in this area that has done testing on the specific type of resilient surfacing in use along with the required depth for safety.

Comments:
An unannounced monitoring inspection was conducted on 04/25/2023 from 10:55am to 12:45pm. There were 20 children ages 3 years old to age 5 years old supervised by 2 staff. The physical plant, outdoor playground areas, programming, attendance, menus, 2 staff records, 3 children?s records, emergency drills, emergency supplies and policies were reviewed. There were no medications onsite. Children were observed participating in lunch and transitioning to nap. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22.1-289.035-B-4
Description: Based on review of 2 staff records, the center did not obtain the results of a criminal history record check from all states in which staff members have resided within the last five years prior to employment.

Evidence:
The record for Staff #2 (date of hire 2/14/19) did not contain documentation of a completed criminal history record check from all states where they resided within the last five years prior to employment.

Standard #: 8VAC20-780-330-B
Description: Based on observation and inspection of the playground, the licensee did not to ensure that the required resilient surfacing surrounding playground equipment with moving parts or climbing apparatus created a sufficient fall zone.

Evidence:
1)The School Age playground has a slide that is 94 inches in height with wood chips measuring at 3 inches in depth at 18 inches from the base of the slide. The required depth of wood chips in this area should be 9 inches.
2)The School Age playground has 8 mushroom shaped climbing pieces clustered in a group on the playground with only 10 inches between them instead of the required 6 feet of fall zone.
