Comments:
The Licensing Inspector initiated and completed an unannounced renewal inspection on 4/13/2023. The inspector was on site from 8:50am until 11:25am.
There were 60 children under the care of 11 staff. The ages of the children ranged from four months to five years old. The children were observed during indoor free play, outdoor play and group time. Five children?s record and three staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the center failed to obtain a copy of the child physical examination before child?s first day or within 30 days after first day of attendance.
Evidence: Child #2 has an enrolled on 10/18/2022, had no documentation of a physical on file during the inspection on 4/13/2023.
Plan of Correction:
Parent was informed day of inspection that the form was needed.
