Comments:
An unannounced complaint inspection was initiated on 07/20/2023. The inspector was on site from 2:05pm until 3:15pm Interviews were conducted with staff and outside Agencies. One children's record and two staff records were reviewed.
Information gathered during the inspection determined the complaint to be invalid. Non-complaint related violations are documented on the violation notice issued to the program.

Based on record review, the center failed to ensure each staff member has a documented TB test or screening every two years.
Evidence: Staff #1 was working day of inspection on 7/20/2023. Staff #1 last TB screening was dated 07/16/2021.
