Comments:
An unannounced monitoring inspection was initiated and completed on 3/26/2024. The inspector was on site from 9:50am until 12:15pm.
There were 55 children, ages 7 months to 8 years old under the supervision of eleven staff. Children were observed during indoor free play, lunch time and naptime. Five children's records and six staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review and interview, the center failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the competition of a fingerprint background check as required by the Code of Virginia.
Evidence: Staff #3, hire date of 3/13/2024 had fingerprint eligibility letter was dated 3/18/2024.
Plan of Correction:
Staff #3 was not in the center until 3-19, however HR will ensure that all fingerprints are done before any training happens.

Description:
REPEAT VIOLATION Based on record review and staff interviews, the center failed to obtain documentation that each child has received immunizations required by the State Board of Health the child can attend the center.
Evidence: 1.The record for Child 3 with an enrollment date of 2/20/2023, did not contain documentation that they received the immunizations required by the State Board of Health during the inspection on 3/26/2024. 2 .The record for Child 5 with an enrollment date of 3/16/2023, did not contain documentation that they received the immunizations required by the State Board of Health during the inspection on 3/26/2024.
Plan of Correction:
The PD will ensure that all children have a copy of their immunizations before enrolled. Child #3 has returned their immunizations and child #5 has until 4-12 to get their physical in or care will discontinue.

Description:
REPEAT VIOLATION Based on record review, the center failed to obtain a copy of the child physical examination before child?s first day or within 30 days after first day of attendance.
Evidence: 1.Child #1, which was enrolled on 1/9/2024 had no documentation of a physical on file during the inspection on 3/26/2024. 2. Child #3, which was enrolled on 2/20/2023 had no documentation of a physical on file during the inspection on 3/26/2024. 3. Child #5, which was enrolled on 3/16/2023 had no documentation of a physical on file during the inspection on 3/26/2024.
Plan of Correction:
The PD will ensure that all children have a copy of the physical before the child's first day or within 30 days after first day. Child #3 physical is in and child #5 has until 4-12 to get their physical in or care will discontinue.
