Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Comments:
A monitoring inspection was initiated and concluded on 5/28/20. The provider was contacted by telephone to initiate the inspection. There were (7) children present and (1) staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed (2) children?s records and 1 staff record submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
Based on child record review, health exams were not obtained for two children within 30 days of a child's attendance. Evidence: Child #1 and #2 began care on 08/12/19. Health exams for both children have not been obtained by the Provider.
Plan of Correction:
Provider will request health exams for both these children as soon as possible given the current health pandemic.

Description:
Based on staff record review, out of state background checks were not obtained for one staff. Evidence: Staff #1 was hired on 2/15/20. a Central Registry search was not requested from two other states that this staff has resided in during the past five years.
Plan of Correction:
Provider will request both of these searches immediately and will forward the results to the inspector for review.
